PART I: WEBSITE PRIVACY POLICY (Squarespace Data)
This section explains how Steamtown Therapies collects data via www.steamtowntherapies.com.
Information Collected Automatically: Our website is hosted by Squarespace. Squarespace automatically collects non-identifying data (IP addresses, browser types, and cookie data) to track website metrics and ensure site security. Information You Provide: We collect personal information (name, email, phone) that you voluntarily submit through our website contact forms. Data Security Limitations: Because standard Squarespace forms are not fully encrypted web environments, transmission of data through our general contact form is done at your own risk. We do not sell or rent your website data to third parties.
PART II: HIPAA NOTICE OF PRIVACY PRACTICES (Clinical Data)
THIS NOTICE DESCRIBES HOW MEDICAL AND MENTAL HEALTH INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Steamtown Therapies is required by federal law (HIPAA) and Pennsylvania state law (including the PA Mental Health Procedures Act) to protect the privacy of your Protected Health Information (PHI).
1. Uses and Disclosures for Treatment, Payment, and Healthcare Operations
We may use your PHI to coordinate your mental health treatment, bill your insurance or process self-pay payments, and conduct routine healthcare business operations.
2. Permitted Disclosures Without Your Consent
Under Pennsylvania and federal law, we must or may disclose PHI without your authorization in the following urgent scenarios:
Mandated Reporting: Suspected child abuse (under PA Child Protective Services Law), elder abuse, or vulnerable adult neglect.
Duty to Warn: Imminent threats of serious physical harm to yourself or an identifiable third party.
Legal Proceedings: In response to a lawful, judge-signed court order. (Note: Standard lawyer subpoenas are heavily restricted under PA mental health statutes without your explicit signed release).
3. Your Rights
You have the right to inspect and receive a copy of your clinical record, request amendments to your records, or request a restriction on how your PHI is disclosed. To exercise these rights, contact our Privacy Officer at your designated practice email.
